Unique Applied Failure Analysis Case Studies

Findings are always intriguing. One wonders what happened in the past/ in the background in order to produce the found faults. Perhaps more important, is what can be done differently to prevent another part(s) / component from reoffending. One question that is often asked is, was there any warning signs that went undetected/ not responded to? During todays meeting we will look at examples of intriguing failure analysis case studies. We will explore what happened in the background that allowed them to develop and then make recommendations on what to do prevent them from reoffending.
Our speaker:
Ralph Baker completed a technology program at NAIT and then apprenticed (HET) with Amoco Petroleum. As a journeyman, Ralph worked for Ferguson Supply before beginning a 22 year carrier with NAIT (Heavy Equipment Programs) and then another 20+ years with Finning Training. The STLE Alberta have also had the pleasure of Ralph being part of our group for over 30 years.
