Application of the Failure Analysis process and the resultant identification of the Most Probable Root Cause of a machine failure provides you with the data needed to support a change; a change that is required to prevent future reoccurrence of that failure. It is bad enough that one failure has occurred. It is much worse if there is a second , especially if you did nothing different to reduce the possibility of the reoccurring failure.
Together let’s look at some parts, pictures, check out some case studies – identify and then apply the data sources available to us as we determine the Most Probable Root Cause.
